PRISM requires the ability to uniformly capture patient information across the four EMUs and store the data in a common format with real-time access by researchers to support cohort identification.

(PRISM) 1) leverage existing expertise and infrastructure at our Center to create a novel, integrated multi-modal data acquisition and management system called MEDCIS (Multi-modality Epilepsy Data Capture and Integration System).

Admission Note: demography, present history, past medical and surgical history, family history, psychosocial history, past and current anti-epileptic, anti-depressant and neuroleptic medications, physical and neurological examinations, evaluations, classification of paroxysmal episodes, and impression and plan.

Progress Note: current admission’s video/EEG, classification of paroxysmal episodes, current anti-epileptic medications, description of recorded seizures, and impression and plan

Discharge summary: It has the purpose of capturing the patient’s personal information and medical histories, keeping track of treatments and medications during hospitalization as well as recording diagnosis, conclusions and recommendations.
